Who is making the claim?
You can submit a claim yourself, or your vet can do it on your behalf. We often recommend letting your vet submit it, as they already have access to your dog’s medical history and invoices - making the process smoother.

If treatment was at multiple practices, submit a separate claim for each. Complete the online form below, and we’ll keep you updated at every step. We may contact your vet to confirm details if needed.
